facetofaceAt Community Health and Wellness, LLC, we believe in curriculum that is tailored to meet the needs of our participants. Through our Life Skills Program: FACE to face, we want to help individuals develop essential skills needed for work, whether voluntary or paid. FACE stands for Focus, Acceptance, Challenge, and Engage. In our program, we not only believe in adaptable programming but also pragmatic approaches through FACE to face interaction. Our program is geared towards high functioning individuals with intellectual disabilities, Autism, and neurological disorders. Currently, we have ongoing programs for individuals ages 14-22 and 22+.


Focus is directly correlated with someone’s interest or activity. When considering work, defined as voluntary or paid, everyone wants to choose a path that complements their natural talents, fits their personal work style, and provides them with personal as well as professional development. Through leisure participation in a 1:1 or group setting, we aim to determine an individual’s interests and/or desires as well as likes and dislikes in regards to activity participation.


Acceptance is the action of consenting to receive or undertake something offered and/or the action or process of being received as adequate or suitable. In any workplace, we believe one’s ability to develop personal coping skills in order to deal with various stressors that come up in a work setting is absolutely necessary for success. In 1:1 or group setting, our staff will provide hands on activities that encourage emotional expression, communication, and personal copings skills in a safe, supported environment.


In any work setting, individuals will face personal and professional challenges, such as work challenges or deadlines, social conflict, mistakes, time management, office bullying, etc.! In FACE to face, we want to work through these challenges! Through role playing, art, and leisure education, we will work to build self-efficacy, locus of control, self-awareness, decision making skills, self-help skills and communication skills in order to appropriately deal with the many challenges associated with any work environment.


In order to learn, an individual must be engaged! During our 1:1 and group programs, our staff works diligently to engage all participants. Whether it’s through the design, adaptation, or facilitation of a program, we will exhaust all efforts to build bridges between ourselves and clients in order to achieve maximum success.

FACE to face Program Essential Eligibility

Community Health and Wellness, LLC has established guidelines to ensure inclusion in all programs by providing quality leisure services in a safe environment.  As a policy, personal care is NOT provided in therapeutic programs, including the FACE to face program.  Participants in the program are expected to:

  • Be able to function in a group setting with a 1:8 staff to participant ratio for up to 2 hours.
  • Be independent in toileting.  Staff will provide verbal prompts to use the restroom and wash hands at appropriate times.
  • Have socially acceptable behaviors.  Participants may not have any aggressive behaviors towards themselves or others.
  • Be independently mobile.
  • Be able to remain with the group 75% of the time.
  • Be independent in dressing in a timely manner.  Staff will provide minimal assistance such as tying shoelaces and putting on coats.  In the case an activity requires a full change of clothes (i.e. swimming), Staff will provide verbal prompts to change, but will not provide assistance in changing.

Community Health and Wellness, LLC programs and activities are designed for group interaction and enrichment, therefore, those participants requiring 1:1 assistance more than 25% of the time or are unable to meet the above essential eligibilities of the program, may provide, at their own cost, an attendant without being charged additional program fees.
